Programs & Services

Communicable Disease Prevention & Control

  • Communicable Diseases-Education, investigation, and follow-up of reportable communicable diseases, potential rabies incidents, suspected food borne illness and disease outbreaks.
  • Immunizations- Provided for infants, children, and adults who are uninsured or whose insurance does not cover vaccine cost. read more
  • Tuberculosis (TB) Screening & Management- Skin testing (mantoux) for assessment of exposure to TB. Education and medication monitoring of persons requiring treatment.

Maternal Child Health

  • Family Home Visiting- Visits to pregnant women, infants and children for assessment, education, support, screening and referral for linking to community resources.
    • The visits focus on education to include preventative health care for healthy pregnancies, mothers, babies, and families; nutrition, breastfeeding, bottle feeding, childbirth, parenting, safety, infant care, growth, and development.
    • Who is eligible? All pregnant women:  first pregnancy, teen pregnancy, high risk pregnancy, history of chronic disease, complications during pregnancy, poor weight gain / high weight gain, interested in breastfeeding, history of preterm labor, multiple births, WIC clients, Postpartum women, infants & children from birth to  3 years, concerns about postpartum depression
  • Nurse Family Partnership- Nurse Family Partnership is a program for women who are having their first baby. A Specially trained nurse will visit in the home every week or two throughout the pregnancy and continue up until the baby is 2 years of age.
    • Have a healthy pregnancy and a healthy baby.
    • Become a better parent.
    • Build a strong network of support for mother and baby; make the home a safe place for baby to live and play; get referrals for healthcare, childcare, job training, and other support services available in the community; find ways to continue client’s education and develop job skills; set goals for the family’s future and find ways to help client reach them.
       
  • Superior Babies (Northern St. Louis County)- Superior Babies Program provides supportive relationships to any pregnant woman with a history of substance use. Superior Babies is committed to helping the mother reach a variety of life-skills and parenting goals to help meet her needs through pregnancy and until the baby reaches two years of age.
    • A client will partner with a licensed chemical dependency counselor and public health nurse. They will help her have the healthiest baby possible by assisting client with remaining drug and alcohol free; provide pregnancy and parenting education; help her find housing, food, support, and assist her to reach her employment and education goals.
    • Who is eligible-Any pregnant woman who wants to have a healthy baby and has questions or concerns about her chemical use.
    • The Programs are voluntary & free of charge.
    • Connecting you with services in the Community.
  • WIC (Women, Infants, & Children)- Offers up to date nutrition education along with nutritious free supplemental foods for pregnant women, postpartum women, and infants and children up to age 5.  Income guidelines apply. read more 
     
  • Follow Along Program- Offers free information and guidance for early childhood growth and development through the mail for families with children 0-36 months of age.  Any parent who is interested can enroll their child who is under 36 months.
     
  • Elevated Blood Lead Case Management- Offers case management, education, referral, and support for families who have children with lead poisoning due to exposure to lead.

Adult Health

  • Long Term Care Consultation- Offers information and resources for alternative living arrangements or in-home services for adults.
  • Minnesota Senior Health Options- Offers Minnesota Senior Health Options enrollees a care coordinator who assists them in getting the health care and related support services they need.
  • Personal Care Assistance- Offers assessment of health needs and authorization of personal care assistance services.

Environmental Health

  • Response to Public Health Nuisance calls. Public education on safety hazards indoor environments and environmental health risks.

Health Promotion

  • Health Education- Presentations available on a variety of health related topics. Consultations about public health issues.
  • Community Partnerships- Provide information and work with community leaders on community assessment and policy development related to health issues. read more
  • Public Health Preparedness- Planning with community partners for coordinated emergency response to natural and man-made disasters. read more
